### **Private Duty Nurse (RN/LPN) – 1:1 Home Care**

### **Job Summary**

Centria Healthcare is hiring compassionate **Registered Nurses (RNs) and Licensed Practical Nurses (LPNs)** to provide **1:1 private duty nursing** in patients’ homes. You’ll deliver skilled, personalized care to pediatric and adult patients in a low-stress, relationship-based setting.

**Client #1: Adult Client**

- **Location:** Ypsilanti, MI

- **Schedule:** 8-Hour Night Shifts (10:00 PM – 6:00 AM)

- **Days:** Tuesday, Saturday

- **Overview:** Seeking a attentive nurse for a stable, home-bound adult client requiring ventilator, tracheostomy, G-tube, and seizure management. Supportive family is on-site to assist and provide orientation. Must be comfortable with small dogs (secured during shifts).

**Client #2: Pediatric Client**

- **Location:** Ypsilanti, MI

- **Schedule:** 12-Hour Shifts

- **Days (8:00 AM – 8:00 PM):** Sunday, Wednesday, Thursday, Saturday

- **Nights (8:00 PM – 8:00 AM):** Monday through Sunday (7 days/week)

- **Overview:** Join the dream team for a playful 1-year-old boy who loves dinosaurs! High-tech pediatric case requiring skilled trach, vent, and G-tube care. Family welcomes male and female nurses and offers both virtual and hospital-based meet-and-greets. Friendly dog in the home.

**Client #3: Pediatric Client**

- **Location:** Ypsilanti, MI

- **Schedule:** 12-Hour Shifts (9:00 AM – 9:00 PM)

- **Availability:** Monday through Sunday

- **Overview:** Rewarding opportunity caring for a sweet infant transitioning home from the NICU. Focuses on TPN administration, central line (Broviac) care, and G-tube management with zero respiratory care needed. Key tasks include TPN disconnects (late afternoon) and evening setups. Work alongside a supportive work-from-home mom in a lively, pet-loving household.

 

**Responsibilities**

- Provide skilled nursing care (trach/vent care, medication administration, catheter care)

- Monitor and document patient condition and vital signs

- Assist with daily living activities (ADLs)

- Communicate with families and care teams

- Maintain safe, high-quality, patient-centered care

**Schedule & Pay**

- Full-time and part-time shifts available
