### **Band 6 Occupational Therapist – Community Stroke ServiceLocation:** Hull & East Riding
**Contract:** Temporary

Our client is seeking an experienced **Band 6 Occupational Therapist** to join their **Community Stroke Service** covering Hull and East Riding. This role involves providing specialist occupational therapy interventions to patients recovering from stroke in the community, supporting rehabilitation and promoting independence.

### **Key Responsibilities:**

- Deliver high-quality occupational therapy interventions for stroke patients in a community setting.

- Complete functional assessments and develop individualized rehabilitation plans.

- Support discharge planning and liaise with the multidisciplinary team.

- Promote independence and optimize patient outcomes through evidence-based practice.
